I had the best diaper bag. It opened out so there was a flat changing mat to change the baby on. It had a removable cover on it, which velcroed in so it could be removed and washed. And really good pockets - it opened like a 't' so the change mat was the main part and then it had two sides where they diapers and other stuff went. And it looked like a normal bag - had pockets on the outside for my own stuff
Only bad thing was it was a boring grey colour.

It would be stylish but practical. Would have good compartments and enough room for a small changing mat, wet wipes, a few nappies a change of clothes the odd toy car etc. But also a good section for my stuff like keys, phone etc.
the other MUST is that it fits over the handle bars of the buggy - in my experience this is the only reason to use a 'changing bag' as we call them in the uk not a normal bag.
for me sippy cup etc go under the buggy/pushchair.

Here's my checklist. I have a two and a half year old and a five month old. My diaper bag is a small Vera Bradley backpack, but I think the larger ones are great; they even have the plastic changing pad someone above mentioned.
1 Wipes Box
8 Size 3 Diapers for my infant (I like to always have a one day supply)
3 Pairs Underwear for my preschooler
2 Full Outfits for my infant
2 Pairs Pants/Shorts for my preschooler
1 Sippy Cup full of water for my preschooler
2 Snacks - Granola Bars or Aunt Anne's Bunny Crackers

It would need an impermeable, washable section (ideally easily removable) to put in the dirty diapers. Maybe with a dispenser of plastic bags.
Also, small pockets for things like diaper pins, cream, etc.
